Cochlear Implant Surgery

Types of Hearing Loss

Hearing loss has been categorized into mainly three sections. This is done depending on the part of the auditory system damaged. The main classifications are-

  1. Conductive Hearing Loss

It occurs when sound is not properly conducted through the outer ear canal to eardrum and the ossicles of the middle ear. It results in reduction of sound level or the inability to hear faint sounds. The major causes are-

  • Fluid in middle ear from colds
  • Allergies
  • Perforated eardrum
  • Presence of a foreign body etc.
  1. SensorineuralHearing Loss (SNHL)

It occurs when there is damage to the inner ear (cochlea), or to the nerve pathways from the inner ear to the brain. It cannot be surgically or medically corrected. The main causes are-

  • Genetic or hereditary auditory problems
  • Head trauma
  • Overexposure to loud noise etc.
  1. Mixed Hearing Loss

Sometimes conductive and sensorineural hearing loss may occur in combination leading to damage of both middle and inner ear. This is known as mixed hear loss.

Am I candidate for Cochlear Implant Surgery?

A prime candidate can be described as:

  • Having severe SNHL in both ears.
  • Not having benefitted from other kinds of high power hearing aids.
  • Having lived at least a short amount of time without hearing.
  • Having good language and communication skills to contribute towards post-surgery therapies.

The above mentioned are the required characteristics for a potential candidate for cochlear implant surgery.

Cochlear Implant Surgery Procedure

The surgery is performed under general anesthetics and lasts to about two to three hours. An incision is made behind the ear so as to get easy access to the important mastoid bone that further leads to the main operative area of the ear. After this step, the inner ear is opened and the implant electrodes are inserted. The receiver-stimulating systemis placed by creating a well in the skull. The external part of the implant includes a microphone, a speech processor, and a transmitter. The internal parts include a receiver-stimulating system and electrodes. A patient needs to stay in the hospital for 7-10 days after the surgery.

Expected Results after Surgery

The outcome of the surgery depends on the age of the patient, previous sound experience, participation in the post-surgery therapies and also the quality of training received after the surgery. The patients were studied based on the age and it was found that children below the age of 5 years had shown significant improvements in catching up with speech therapies and were able to speak and understand the same level as normal children.

Children between the age group of 5-10 also had shown significant increase in the verbal and linguistic therapies but lagged behind the children under 5 years of age.

Children above 10 years of age exhibited lower amount of result in grasping the linguistic and verbal therapies and lagged much behind other two age groups.

This study shows a definite pattern that as the age of the patient increases, the chances of coping up with the post surgical therapies decreases. Hence it is necessary to identify the hearing loss at an early stage and to get proper treatment or else it will be difficult for them to cope up with the surgery and its effects.
