ICSI Treatment

Intra cytoplasmic sperm injectionICSI is essentially a treatment for male infertility and is known as Intracytoplasmic sperm injection. This treatment is often used as a part of in vitro fertilization treatment to help childless couples to conceive a child. This is the most successful of all treatments for infertile men and is used in almost half of all IVF cases. In ICSI treatment only one sperm is required which is then injected into the egg and the fertilized egg is transferred to the female uterus.


There are different causes of male infertility:

  • The sperm is unable to reach the egg
  • The sperm is able to reach the egg but cannot fertilize it
  • There is a very low sperm count
  • There is a large number of abnormally formed sperm resulting in poor motility meaning the sperm is unable to swim properly
  • Sperms which cannot be ejaculated but it is possible to collect them from the testicles or the duct where they are stored. This is usually the case where the man has had irreversible vasectomy or injury.
  • Spinal cord injuries or diabetes causing problems with erection or ejaculation

In case you have already tried IVF but there are not enough eggs then you can opt for ICSI.

To whom ICSI is suitable for?

Any couple that is otherwise healthy and has male infertility problems is eligible for this treatment. However, it is necessary to ascertain that there are no genetic deformities which can be passed on to the offspring from the parents and for this a blood test of both the partners is conducted.

ICSI Procedure

The female is given fertility drugs in order to stimulate the production of eggs and when these eggs are ready to be collected, both the partners will undergo separate procedures. The male can ejaculate into a cup for a sperm sample and if there is no sperm in the sample it can be extracted from the male under local anesthesia. There are two methods of collecting the sperms from the male –

  • Sperm is collected from the epididymis and this procedure is known as percutaneous epididymal sperm aspiration
  • Sperm is collected from the testicles in a procedure known as testicular sperm aspiration

The retrieved sperms are frozen and now the eggs from the female are collected with the help of a fine hollow needle. The doctor uses an ultra sound to locate the eggs and the embryologist then isolates the individual sperm and injects it into the eggs. After two days the fertilized cells develop into balls of cells known as embryos. After this the same procedure as that used in IVF is adopted and one or two embryos are transplanted into the uterus using a thin catheter. Embryos are usually transferred two to three days or five days after fertilization. In presence of favorable circumstances the embryo will get attached to the wall of the uterus and continue to grow.

Success Rate of ICSI Procedure

The success rates of using ICSI in combination with IVF are higher than if only IVF is used. A lot of things depend on the exact fertility problem and age. In younger women the eggs are usually healthier and provide a higher chance of success.

The percentage of successful cycles of ICSI:

  • 35% for people under 35
  • 29% for people between 35 and 37
  • 21% for people between 38 and 39
  • 14% for people between 40 and 42
  • 6% for people between 43 and 44
  • 5% for people over 44

How Long Does ICSI Treatment Last?

Completing one IVF with ICSI requires about 4 to 6 weeks because the ovaries of the female take some time to respond to the fertility drugs and time is also required for the eggs to mature. While taking the medication the female will have to visit the doctor every two to three days for blood work and ultrasound. On the day when the eggs are retrieved, both partners will spend several hours at the clinic for the egg and sperm collection. Then after three or five days the female has to go to the doctor’s clinic again to have the embryo transferred. The first pregnancy test is taken about two weeks after this.
