In-Vitro Fertilization(IVF)

What is In-Vitro-Fertilization?

In-vitro fertilization, an advance technique for conception is commonly called as ‘test tube baby’ and is a part of ART (Assisted Reproductive Procedure). The treatment procedure was developed by Robert G. Edwards, who is a Nobel Prize winner in Physiology or Medicine.

In-Vitro Fertilization (IVF)

The procedure requires continuous monitoring of the woman’s ovulatory system. The ovum and sperm are manually combined in the IVF laboratory dish, fertilized and transferred directly into the patient’s uterus. The intention is to establish a successful pregnancy.

Why is IVF Needed?

It is an ideal procedure for the couples facing fertility problems. They may have tried other methods such as fertility medications, surgical technique, and artificial insemination, but it may not have worked really well.

This procedure works best for a woman who has damaged or blocked Fallopian tubes, endometriosis, ovulation problem, fibroids or certain hormonal problems. If the woman’s reproductive system is fine and the male partner has issues like impaired sperm production or function, inability of the sperm to penetrate/ survive in the cervical mucus, poor sperm count, hereditary factor and azoospermia then IVF is the ideal solution.

  1. For monitoring follicular development Ultrasound and hormone assessments are done.
  1. Once the follicles have reached the appropriate size and matured then HCG is administered.
  1. Eggs are retrieved after 30-36 hours after the HCG injection
  1. Performed under intravenous sedation, follicles are removed using a needle.
  1. Eggs are cultured in an incubator until insemination
  1. Sperm cell is injected into the fertilized egg or zygote using Intracytoplasmic Sperm Injection or (ICSI) procedure
  2. If the quality of the embryo is good then the doctor may hatch or recommend Pre-implantation genetic testing methods before embryo implantation. (Only for older women)
  3. The embryo is implanted using long tube called a catheter by inserting it through the cervix in the vagina, directly into the uterus
  4. After implantation, the woman should follow the doctor’s instructions.

Embryo and IVF cycles:

The only single embryo transfer is done in a woman under 38 during her 1st IVF cycle. In 2nd cycle, if more than one top quality embryos are present, then a single embryo is transferred. In the 3rd cycle, not more than 2 embryos should be transferred.

In 1st and 2nd full cycles, women aged 37-40 years should have single embryo transfer. Double embryo transfer can be considered for women aged 40-43 years. The multiple embryos may carry a risk of multiple pregnancy and births.
